We believe hospice should feel different.

If you don’t want to know the doctor helping guide your care — don’t come to us.
If you want your visits to feel rushed — don’t come to us.
Because that is not the kind of hospice we have created.
Gracia was created out of frustration with what we believe has been lost in the traditional hospice model.
We see an industry that has become transactional. Where patients have become census numbers. Where families can feel like they are navigating an enormous system on their own. Where caregivers are rushed from one visit to the next and relationships become secondary to productivity.
We wanted something different.
So we built Gracia around a very simple belief:
People come first.
We want to know you.
When you choose Gracia, we do not want you to disappear into a system.

We want to know your family.
We want to understand what matters to you, what you are afraid of, what you hope for and what a good day looks like in your home.
Our job begins by listening.
Only after we understand your wishes, preferences and priorities can we provide the kind of guidance and care your family actually needs.
Hospice should never feel like something being done to you.
It should feel like a team walking with you.
We want you to feel like a priority.
Families should not have to wonder whether someone will call them back.
They should not feel guilty for asking another question.
They should not have to fight through layers of bureaucracy when something changes.
At Gracia, your priorities become our priorities.
We intentionally want to remain close enough to the families we serve that we can respond, adapt and make decisions based on what is happening in your home—not simply what is easiest for the organization.
Because when someone you love is nearing the end of life, being treated like a priority should not feel exceptional.
It should be the standard.
We believe relationships matter.
Consistency matters.
Trust matters.
Seeing familiar faces matters.
There is comfort in opening your front door and recognizing the nurse standing on the other side.
There is comfort in not having to explain your entire story again.
There is comfort in knowing the people caring for your loved one already understand what has happened, what has changed and what your family needs.
We want our patients and families to actually know their hospice team.
And we want our hospice team to know them.
That includes the physician helping guide their care. The physician will adapt to your personal needs, priorities and preferences.
Healthcare has become increasingly disconnected. We believe hospice should move in the opposite direction.
Hospice should feel personal.
We don't believe meaningful care should feel rushed.
Some visits will be straightforward.
Others won't be.
There will be days when a family needs an extra conversation.
An extra explanation.
An extra few minutes sitting beside the bed, providing guidance on what the coming hours or days might look like and giving the family time to ask questions.
An extra moment to process what is happening.
Care cannot always be reduced to a schedule because symptoms don’t appear between the hours of 8am – 5pm.
We believe in empowering our team to respond to the person in front of them, rather than simply watching the clock.
If extra is needed, we want to provide extra.
Because this may be another visit on a schedule for a healthcare organization.
But for the family experiencing it, this is their life.
We care for more than the patient.
Hospice affects an entire family.
There are physical needs.
But there are also emotional needs.
Mental needs.
Spiritual needs.
Cultural needs.
Questions.
Fears.
Exhaustion.
Grief.
And sometimes simply the need for somebody who understands what is happening and can say, “Here is what you can expect next.”
That is why Gracia believes in whole-family care.
We want families to feel informed, included and supported throughout the journey—not just when there is a crisis. Our Core Values describe care as physical, mental, emotional, spiritual and cultural because we believe ignoring any of those pieces leaves care incomplete.
We don't measure success only in numbers.
Of course we have to run a responsible organization.
But we never want the business of hospice to overshadow the purpose of hospice.

Our goal isn't simply to help someone die.
Our goal is to help families live their lives, together, in the way that matters most to them for as long as possible. That idea is at the heart of Gracia's Core Values: proactive communication, education, collaboration and helping patients and families feel informed, empowered and supported.

If you want to be anonymous, we're probably not the hospice for you.
